CoordinateOperationFactory shall apply vertical transformations

Description

A transformation between WGS84 geoid and ellipsoid has been implemented in the geotk-referencing3D module, but is not yet used by CoordinateOperationFactory: user still need to instantiate the MathTransform explicitly.

We need to improve CoordinateOperationFactory in order to apply the vertical transformation, when possible.
